I really enjoyed completing the medication reconciliation project! I felt like it helped me apply the knowledge I have acquired through class to a real life siltation and it felt good to be able to complete an assignment like this. While compiling information and assessing each specific medication for my client I discovered a few safety concerns that I determined I wanted to discuss with my client. I found one medication interaction, the client was taking Brilinta and Aspirin, these medications are often prescribed together, but too much Aspirin could cause a decrease in the effectiveness of Brilinta and that could lead to an increase bleeding risk. Because of this it is important that my client does not take more than 100 mg of Aspirin per day and would need to be cautious using over the counter medications, since many of them contain Aspirin. I also emphasized to my client another safety concern, that they are at an risk for increased bleeding and I explained to them how they need to be cautious and limit any activities that could lead to bleeding. Also, that they needed to talk with their provider before stopping any of their medications. Lastly, I really wanted to emphasize some important health promotion tips that I felt my client could really benefit from, such as a proper diet, adequate exercise, reduced stress, and taking precautions to decrease the risk of bleeding, as an example, using a soft brittle toothbrush. I felt that this activity really improved my relationship with my client and we were able to build a stronger trust while talking about such personal topics such as health history and the client’s current medications. I felt like my client really respected me and listened to everything I had to say. I am not concerned about my client’s understanding of anything we discussed due to their willingness to learn, their occupation; he is a nurse anesthetist and knows a lot about his condition and treatment plan, and his age in relation to understanding also did not concern me. My client was fully educated on each medication they were taking and had a basic understanding of what each medication did. This is extremely important because the more knowledge the client has about their medications, the better decision they will make in regards to their medications. Overall, I feel confident in my clients understanding of their medication and treatment plan and feel confident in their safe medication administration.
